The MAC Request Form

The form below MUST be completed to coordinate use request for calendar dates and facility use.  If you would prefer to download the form, please click here and return it to MAC office as early as possible before your event.  All requests are reviewed and granted on a "first come, first served" basis.  First Baptist Church Woodway reserves the right to reschedule when church requirements deem it nesessary. Building use fees will be discussed when a MAC staff person confirms your reservation request.

Most events are self set up and clean up.  Room(s) or area(s) must be returned to original condition.  If MAC staff are required to set up and/or tear down, additional fees will be required.  If you are reserving the Party Room, you may share the gym with other groups.

Note: *There is a $50.00 utility and clean up fee for the use of inflatables in the Gym.
